Payment is due at time of booking. Patients can book online, by phone, or walk in.

On the day of the appointment, the patient should bring any pertinent documentation pertaining to the diagnosis when seeing the physician. 

After booking, an email is sent containing the forms. They should be completed online then submitted electronically. 

After the state fee is paid, it take about 5-7 business days for the state to email the patient their approval letter. at that time patients can use the email as a temporary card until the card comes in the mail

On the day of the appointment, the patient brings an ID & any pertinent documentation pertaining to the diagnosis (if any)to see the physician.
